State Inspectors Cite Patapsco Healthcare in Randallstown for Repeated Deficiencies Across Multiple Surveys Involving Resident Care, Safety and Regulatory Compliance

A review of multiple Maryland Office of Health Care Quality (OHCQ) inspection reports obtained by Report Annapolis News documents a series of regulatory deficiencies cited at Patapsco Healthcare, located in Randallstown, between October 2024 through February 2026, involving resident care, resident rights, life safety requirements, environmental conditions, documentation, abuse investigations, and facility operations. The inspection reports reviewed span from October 2024 through February 2026 and include both Life Safety Code surveys and health complaint investigations conducted by state regulators. During each inspection, surveyors reviewed resident records, interviewed staff, residents and family members, observed facility operations, and evaluated compliance with both federal and state regulations governing long-term care facilities.

During a recertification survey completed on February 9, 2026, state surveyors determined Patapsco Healthcare was not in compliance with federal long-term care requirements after reviewing resident records, investigating multiple complaints and facility-reported incidents, and conducting interviews and observations throughout the facility.

Among the deficiencies cited were findings that the facility failed to consistently protect residents' rights and dignity. Surveyors documented concerns involving staff entering residents' rooms without knocking or requesting permission, residents who were dependent on staff for activities of daily living remaining in bed for extended periods, delays in returning residents' clothing from laundry services, and concerns involving privacy and dignity during resident transfers and personal care. Surveyors also documented environmental concerns, including damaged ceiling tiles, cluttered resident rooms, inadequate storage for residents' personal belongings, and visible dark and gray debris in ceiling vents located in hallway areas.

The February 2026 survey also cited the facility under federal requirements related to abuse prevention and investigation. According to the inspection report, surveyors reviewed a facility-reported incident involving allegations initially described as inappropriate touching and potential physical abuse.

 Inspectors identified concerns with the facility's investigative process, including documentation indicating law enforcement was not contacted and that certain agencies were not notified following the initial report. The report states the facility implemented corrective actions, including one-to-one supervision for the resident identified as the alleged perpetrator and additional staff education. The inspection report does not conclude that criminal conduct occurred but instead cites deficiencies related to the facility's compliance with federal abuse prevention, reporting, and investigative requirements.

A separate complaint investigation completed in September 2025 likewise concluded the facility

 was not in compliance with federal long-term care regulations following an investigation into numerous resident complaints and facility-reported incidents. State inspectors cited deficiencies involving notification of residents' representatives regarding significant changes in residents' medical conditions and treatment plans, failure to complete required discharge summaries within required timeframes, deficiencies in investigating resident injuries, and concerns involving nursing services.

Separate Life Safety Code inspections conducted in October and November 2024 identified numerous fire safety and physical plant deficiencies throughout the facility. 

According to state inspection reports, surveyors cited Patapsco Healthcare for deficiencies including incomplete emergency preparedness planning, failure to conduct required emergency preparedness exercises, missed quarterly fire drills, inadequate documentation of annual fire door inspections, corridor doors that failed to properly self-close and latch, electrical safety concerns, deficiencies involving enforcement of the facility's smoking policy, improper placement of fire extinguishers, smoke compartment maintenance concerns, and failure to document required electrical receptacle testing. 

State inspectors noted that many of these deficiencies had the potential to place residents and staff at increased risk during a fire or other emergency.

Each inspection report includes a Plan of Correction submitted by Patapsco Healthcare describing steps the facility stated it would take to address the cited deficiencies. Those corrective measures include staff education and retraining, revisions to facility policies, increased auditing and quality assurance reviews, maintenance repairs, environmental improvements, enhanced documentation practices, and ongoing monitoring by facility leadership. 

The reports reviewed by Report Annapolis News reflect regulatory findings made by the Maryland Office of Health Care Quality during specific inspections and complaint investigations. Regulatory deficiency citations are administrative findings regarding compliance with state and federal standards and are separate from findings of civil liability or criminal wrongdoing.
